Health & Counseling Services Consent Forms 2025-2026

As required by Senate Bill 12 and with guidance from the Texas Association of School Boards, Redwater ISD is requesting your annual consent for your child to receive routine school-based physical and mental health-related services from qualified school staff, including nurses, athletic trainers, and counselors.

This form allows you to provide your consent for your child to access school counseling and health services as needed during the school year. A new form must be completed for each student.
